The Family Support Network connects families and young people at risk with support services

Communicare is the lead agency for the Fremantle-Rockingham Family Support Network (FSN), supporting families by assessing their needs, identifying risks, finding early interventions, and connecting them with the support services they need to be happy and healthy together. The FSN helps in the prevention of child abuse and neglect, building resistance for families, and keeping children out of out-of-home care whenever possible. Communicare deliver this program in partnership with the Langford Aboriginal Association.

The FSN provides two streams of support to families assisting them parent their children safely and divert them from the child protection system.

The Assessment and Coordination stream provides early intervention support for families and young people.

The Intensive Case Management stream only accepts referrals from the Department of Communities and is for families who require intensive support to keep their children safely at home.

Who can it help

The FSN aids vulnerable children, families and young people aged up to 25 years, experiencing multiple and complex situations such as:

  • Mental Health
  • Family and domestic violence
  • Challenges with substance abuse
  • Homelessness or risk of becoming homeless
  • Family conflict
  • Parenting difficulties

Next Steps

Referrals are accepted from the Department of Communities, FSN partner agencies, non-partner agencies in the community and directly from families themselves.
